Consultation
Our consultation service </span> provides personalized support to help individuals navigate the complexities of housing options. We understand that each person’s situation is unique, and our experienced team is here to listen, assess needs, and offer tailored advice. Whether you are exploring your options, facing challenges, or simply seeking guidance, our consultations are designed to equip you with the information and resources necessary to make informed decisions about your housing.
Transitioning
The transition to stable housing can be daunting, especially for those with disabilities. Our Transitioning service focuses on facilitating a smooth move into new accommodations. We assist clients with everything from locating suitable housing to coordinating logistics and providing emotional support throughout the process. Our goal is to ensure that each individual feels secure and confident as they make this important life change.
Sustaining
Once you have found stable housing, our Sustainment service continues to support your journey. We offer ongoing assistance to help individuals maintain their housing and address any challenges that may arise. This includes regular check-ins, resource referrals, and skills training to foster independence. We believe that everyone deserves a place to call home, and we are committed to helping you sustain that home for the long term.

Eligibility
- Must be 18 years or older
- Eligible for Medical Assistance (MA) program
- Must have a documented disability or disabling condition, which includes:
- Substance use disorder and enrolled in a treatment program or on awaiting list for a treatment program.
- Learning disability as determined by the Department of Human Services(DHS).

Mental health condition, substance use disorder, or physical injury that required a residential level of care and is now transitioning to the community. - Mental illness.
- Aged, blind, or a disability as described under Title II of the Social SecurityAct.
- Individuals who Injury or illness that is expected to cause extended or long-termincapacitationeligible for Supplemental Security Income (SSI).
- Developmental disability or related condition.
- Must be experiencing housing instability, evidenced by one of the following riskfactors:
- Homeless
- Currently transitioning or has recently transitioned from an institution orlicensed or registered setting
- At risk of homelessness
- At risk of institutionalization
- Eligible for the following waivers:
- Elderly Waiver (EW)
- Developmental Disability (DD)
- Community Access for Disability Inclusion (CADI)
- Community Alternative Care (CAC)
